Application Fees Must be Made Payable to the Housatonic Valley Health District.

*Please note that the application forms have been revised. Applications that do not relate to Subsurface Sewage Disposal Systems can all be applied for via the “Application for Approval for Building Addition-Conversion” application form.
Incomplete applications will be returned.
When submitting applications to the Health Department please include a separate check for each individual application you have filled out, and when submitting applications to multiple departments simultaneously please include a separate map or diagram of the proposed construction for each department that you are applying to.
Satellite images and photographs will not be accepted by the Health Department, please include the proposed construction on an As-Built drawing with clearly labeled distances between the well, septic, and other relevant structures.
